Information for Student Athletes

Athletic Physical Form

All students who intend to play intercollegiate sports must have a physical exam performed by their doctor before they are allowed to participate in the athletic program. Please complete the athletic physical exam form and return it to the Athletic Department by August 15, 2008 (for fall sports) or September 5, 2008 (for all other sports).

Intercollegiate Sports Insurance Coverage

First Level of Coverage

A. The student’s primary insurance is billed for any injuries sustained. In the event that there are items that are not covered after the primary insurance pays the first $500, those bills will be submitted to the Basic Sports Accident Insurance Policy through the Medaille College Athletic Trainer.

Second Level of Coverage

B. Intercollegiate Sports Injury Coverage under the Basic Sports Accident Insurance Policy:

-Provides up to $75,000 of coverage ($500 deductible) payable at 100% of covered expenses, on an excess basis. Coverage provided through the AIG Insurance Company.

Also, covers the following:

- Claims payable for 104 weeks.
- Pre-Existing Conditions.
- Expanded Medical Conditions (“wear and tear” type injuries, (ex: rotator cuff, stress fracture, bursitis, tendonitis, etc.).
- HMO/PPO Denials.

Third Level of Coverage

C. Intercollegiate Sports Injury Coverage under the NCAA
Catastrophic Sports Accident Insurance Policy:

-Provides up to $10,000,000 of coverage ($75,000 deductible) payable at 100% of covered expenses, on an excess basis. Coverage provided through the NCAA.
