All students who intend to play intercollegiate sports at Medaille must have the following documents on file with the Department of Athletics. Please submit all information to Laura Edholm, associate athletic director, at Medaille College, 18 Agassiz Circle, Buffalo, NY 14214.
Health Insurance Card
A copy of the front and back of your health insurance card.
Athletic Physical Form
All students who intend to play intercollegiate sports must have a physical exam performed by their doctor before they are allowed to participate in the athletic program. If you are unable to see your own doctor for the physical, please contact the Department of Athletics. Physicals will be conductd at Medaille on specific dates for a fee.
Forms are due August 10, 2012 for fall sports, and September 15, 2012 for winter and spring sports.

The instructors were knowledgeable and welcomed class discussions, with
respect for each student's contribution. I've learned how to be a more
effective leader. My capstone class gave insight on how to combine all
the education learned to operate a business. That's when I realized how
much I had sharpened my knowledge.
