You MUST inspect the Vivarium on three separate days during your assigned week.

Each of the YES/NO columns is for one day of inspection.

If you make a mistake, draw one line through the error, initial the error and write your corrections.  DO NOT ERASE, OBLITERATE OR WHITE-OUT ANY ENTRY!!

This form must be filled in blue or black ball-point pen.

At the END of the week (after your last inspection), sign the bottom of the form with your name under "Inspected by", write student under "Title" and date your signature.
