It is the policy of Medaille College, to provide, when possible, all students and guests equal access to programs, services, and activities sponsored by the institution.  This form is to be used for a grievance with academic and/or physical accommodations in accordance with the Americans with Disabilities Act.  Medical supporting documentation may be required in the event of an institutional review.

All grievances will be reviewed by the Accessibility Services Coordinator, 504 Coordinator (for facilities issues), Academic Support Center Director and Sr. Director for Instructional Support.
