Please submit this completed form to request an ID card replacement if your card has been damaged, lost or stolen. You must acknowledge that if you need an additional ID card replacement at any time after the initial replacement, there will be a $10 charge to your student account.

When you submit this form, you will receive an email confirmation. You must print the confirmation and bring it to Campus Safety to receive your ID card replacement.

If you have a meal plan, please visit the Office of Student Affairs (203 Sullivan Center) to activate your plan on your new card.
